Site Observation:
Written procedures are not maintained when using Time as a Public Health Control (TPHC) (Potentially hazardous food (PHF) items - cheese slices and grated cheese. The food items are time marked however the procedures are not available). [CA] When time only, rather than time and temperature are used as a public health control, records and documentation must be maintained.
Major Violation Cited - K14M - Food contact surfaces unclean and unsanitized
Site Observation:
Observed black mold like substance on the side walls in side the ice machine. [CA] Clean and sanitize the ice machine following manufacturers directions. [SA] Discontinue using ice machine until it is thoroughly cleaned and sanitized.
Minor Violation Cited - K27 - Food not separated and unprotected
Site Observation:
Observed bread packages stored next to mop rack at the kitchen back area. .[CA] All food shall be separated and protected from contamination. Do not store bread at the mop area to prevent contamination.

Site Observation:
Observed nuggets box stored on floor at the walk in freezer. [CA] Food shall be stored at least 6" above the floor on approved shelving.

Site Observation:
The inside of the microwave oven has accumulation of food debris. [CA] The inside of the microwave oven shall be cleaned and sanitized of food stains, grimes and debris.
Minor Violation Cited - K40 - Wiping cloths: improperly used and stored
Site Observation:
Measured the quat sanitizer concentration at 0ppm in sanitizer bucket holding wiping cloths in prep station. [CA] Maintain quat sanitizer concentration at 200ppm in sanitizer bucket holding wiping cloths.

Site Observation:
Potentially Hazardous Foods (PHFs) such as cheese slices, diced tomatoes at cook line subject to Time as a Public Health Control (TPHC) are not destroyed after 4 hours (PHF has time coding that shows product time has passed).[CA]PHF subject to Time as a Public Health Control (TPHC) shall be destroyed after 4 hours.

Written procedures are not maintained and available for food that has been prepared, cooked and refrigerated when using Time as a Public Health Control.[CA]When time only, rather than time and temperature are used as a public health control, records and documentation must be maintained.
