Site Observation:
Drain flies touching slices of chocolate and carrot cakes. [CA] Discard food items which flies have made contact with. [COS] VC & D.
Major Violation Cited - K14M - Food contact surfaces unclean and unsanitized
Site Observation:
Major- Quat sanitizer solution, in which utensils were submerged at warewashing sink, measured at 0 PPM. [CA] Provide 200 PPM quat solution for warewashing sink. [COS] PIC adjusted quat to 200 PPM.

Minor- High temp warewashing machine after complete cycle measured at 150F; ran five consecutive times and used three different thermolabels. [CA] Provide at minimum 160F water temperature at high temp warewashing machine. [COS] PIC setup warewashing 3-comp sink with 200 PPM quat for manual dishware sanitizing.

A few drain flies in the dessert buffet area. [CA] Eliminate vermin and maintain facility free of vermin.

Site Observation:
1) Crushed red pepper in the dry storage room does not have a lid. [CA] Provide tight fighting lid.

2) Open bag of flour in the dry storage room. [CA] Place flour inside of container with tight fitting lid.

Site Observation:
1) Metal ice scoop is cracked. [CA] Replace ice scoop. [COS] PIC removed ice scoop from service.

2) High temp warewashing machine gauge reads approximately 180 F during final rinse. Three thermolabels used on metal containers throughout complete cycles in this machine indicated temperature did not reach 160F. [CA] Repair high temp warewashing machine to provide at least 160 F during final rinse.

3) Quat dispenser at warewashing is being used with empty quat sanitizer container. [CA] Maintain equipment in working condition.

Site Observation:
Containers with food are stored ontop of clean utensils. [CA] Protect clean utensils from contamination.

Site Observation:
1) Employee backpack ontop of food prep surface. [CA] Store employee personal items in approved employee item storage location.

2) Open employee's beverage in kitchen prep area. [CA] Provide tight fitting or lid with straw for employee drinking containers.

3) No employee lockers are available. Per PIC, lockers were removed. [CA] Provide employee lockers and secure to rigid structure.

Site Observation:
1) Ceiling in the dishwashing area is peeling, has black mold-like substance, and rust in some areas. [CA] Clean and repair ceiling. *repeat violation*

2) Floor sink under beverage service area has glove inside. [CA] Remove glove and any debris from floor sink.

3) Missing floor tiles in kitchen. [CA] Install floor tiles.

4) Metal coving damaged by walk-in refrigerators. [CA] Repair coving.

Site Observation:
Steamed fish at buffet hot holding measured at 126F. [CA] Maintain PHF hot at 135F or above. [COS] Steamed fish discarded. [NOTE] All other PHFs at hot holding measured at 140-160F. Steamed fish is only food item out of hot holding temp.

Site Observation:
- Underside of milk dispenser and surface behind soda dispenser ice chute are unclean with milk residues/splashes and mildew. [CA] Maintain equipment clean.
- Plastic protection film on new refrigerator and high temp dishwasher booster heater. [CA] Remove plastic film on equipment.

Site Observation:
- Black mold like substances on wall behind dishwahser.
- T-bars on dishwashing room ceilings are rusted. Cookline ceiling tiles are stained. Some ceiling tiles are acoustic panel.
- Some cove base stainless panel outside walk-in coolers are deteriorated. [CA] Maintain walls, ceilings and floors clean and in good repair.

Major Violation Cited - K07M - Improper hot and cold holding temperatures
Site Observation:
- At salad reach-in cooler, all food items (both PHFs and Non-PHFs) measured at 49-52F. Per Nancy, these are prepared yesterday. [CA] Maintain PHFs cold at 41F or below. [COS] Nancy discarded PHFs (i.e. salad w. mayo) into garbage. Non PHFs (i.e. cut cucumber, shredded carrots, etc.) are moved to working walk-in cooler. These desserts were put out to buffet area <2hrs.
- At buffet dessert display cooler, all desserts are measured at 59-60F. Ambient temperature measured at 60F. [CA] Maintain PHFs cold at 41F or below. [COS] Tray of ice provided under desserts.
Major Violation Cited - K14M - Food contact surfaces unclean and unsanitized
Site Observation:
Low temp dishwasher failed to provide 50ppm chlorine sanitizer. [Note] When inspector entered the warewashing room, dishwasher water was drained out and dishwasher was not running. Per Nancy, they empty out dishwasher, clean inside dishwasher (i.e. food particles), refill water and re-start dishwasher every 2-3 hours (per service provider's recommendation). Asked Nancy to fill water in dishwasher and re-start as they normally do. After dishwasher re-started and running, noted 10ppm chlorine -> 50ppm chlorine then consecutively 0ppm chlorine residual -> 50ppm ->0ppm. Test over 20 times. *** This is a repeated violation. And already notified GM and failure to comply may result in enforcement action and/or fines. [SA] 3 compartment sink set up with 200ppm QUAT. Nancy will call for service. Until fixed, use 3 compartment sink for sanitizing all utensils and equipment.

Site Observation:
- EXCESSIVE water accumulation inside reach-in salad prep cooler. Ambient measured at 49F. Also, rubber gasket is coming off from door. In addition, dessert cooler at buffet area measured at 60F ambient temperature. [CA] Maintain refrigeration units in proper working condition. [Note] Nancy called for service.
- Blood is dripping from metro rack inside meat walk-in cooler. [CA] Maintain equipment in clean condition.
- #### 0ppm chlorine residual at low temp dishwasher. [CA] Maintain equipment in good repair. [Note] Both inspector and assistant manager, Nancy, tested over 10 times consecutively. Failed to obtain 50ppm chlorine residual.

Site Observation:
- Dirty knife stored in clean knife holder. In addition, knife is stored inside HOOD OVERHANG. (This is a major safety concern.) [CA] Store clean and sanitized knife and sanitary manner (i.e. wall mount magnetic knife holder).

- One ceiling panel above tea boiler is missing. Another ceiling panel is sagging. [CA] Maintain ceiling panels in good condition. Provide hard ceiling panels in kitchen.
- Yellow/brown discoloration on ceiling panels in deep frying area. [CA] Maintain ceiling panels clean.

Major Violation Cited - K14M - Food contact surfaces unclean and unsanitized
Site Observation:
Warewashing machine measured with chlorine sanitizer reading at 0 ppm.[CA] Warewashing machine shall have chlorine sanitizer measured at 50 ppm. [COS] General Manager detected air gap and after fixing air gap in hose, chlorine measured at 50ppm.
****** 0 ppm chlorine is a repeated issue at this facility and office hearing conducted to address this issue. However, during time of this inspection, dishwasher had 0ppm chlorine reading again. During office hearing, management agreed to provide daily chlorine reading log, however, dishwasher log not maintained. ALSO, manager has been using QUAT test strips (Hydrion QUAT CHEK yellow color strips) to measure Liquid Sodium Hypochlorite (chlorine).
****** Will consult supervisor to address this repeated major violation.

- Underside of milk dispenser, hot cocoa dispenser and soda dispenser has black mold substances and excessive sticky residues. [CA] Maintain equipment in clean condition.
- Rack inside salad cooler reach-i cooler has mold like substances. Rack inside meat walk-in cooler has blood dripping from the rack. [CA] Maintain non-food contact surfaces clean.

Water is leaking from kitchen handwashing sink P-trap. [CA] Fix the leaky plumbing fixture.

Site Observation:
- Several ceiling panels in kitchen are perforated ceiling panels. [CA] Provide smooth and easily cleanable ceiling material (i.e. hard ceiling panels.)
- FRP panel on wall adjacent to dishwashing area is coming off and rotten wood wall is exposed. [CA] Maintain walls in good condition.
