Site Observation:
Lack of paper towel in dispenser at one of the handwash sink in the food prep area and at the service line. Additional handwash sink in bakery area around the corner. [CA] Provide single use paper towels in dispenser at handwash station at all times.

Site Observation:
Measured baked chicken in warmer at 125F. Measured chicken gravy and beef grave at 93F and 100F, respectively. [CA] Maintain hot PHFs hot at 135F or above.

[COS] Operator discarded products at time of inspection.

Site Observation:
Meat slicer is not clean to sight and touch. Per operator, meat slicer was not in use nor planned to be used for remainder of the day. [CA] Equipment shall be washed, rinsed and sanitized after each use or after 4 hours if continuously used.

Site Observation:
Floor drain in front of steamer is slowly draining but there is no sign of sewage on the floor. Floor drain in front of ice machine was slowly draining after cookline drain was cleared. Both drains are located in separate areas. [CA] Liquid waste shall be disposed through the approved plumbing system and shall discharge into the public sewerage or into an approved private sewage disposal system.

[COS] Operator was able to clear both clogged drains.

Lack of soap at employee restroom handwashing sink. [CA] Provide soap in wall mount dispenser at all times. [COS]
Major Violation Cited - K07M - Improper hot and cold holding temperatures
Site Observation:
Various PHFs in meat walk-in measured at 52-53F. GM and employee confirmed these were placed in this walk-in cooler yesterday. [CA] Keep PHFs cold at 41F or below. [COS] VC&D all PHFs which were 52-53F. See VC&D report.

Site Observation:
> Yellowish slime build up inside ice machine. Inspector swiped with alcohol wipe to confirm. [CA] Clean and SANITIZE food contact surfaces. Empty out ice, thoroughly clean and SANITIZE.
> Heat at HIGH TEMP dishwasher measured at 150F at food contact surfaces after running for 5 consecutive times. [CA] Provide min. 160F at food contact surfaces. [SA] Temporarily, you may SANITIZE all clean dishes and utensils at warewashing sink with 200ppm QUAT.

Broken tiles at several spots in kitchen. Exterior cove base of walk-in cooler damaged. Floors are not clean with excessive food debris and gray water. Ceiling panel above high temp dishwasher is damaged/cracked. [CA] Maintain floors, walls and ceilings clean and in good repair.

05/18/2016 - ROUTINE INSPECTION
Major Violation Cited - K22M - Sewage and wastewater improperly disposed
Site Observation:
Floor sink across soup kettle is over flowing when both prep sinks are draining. [CA] All liquid wast must drain to a fully functioing sewage disposal system. [NOTE] Two prep sinks are draining to one floor sink. Manager is aware that when BOTH prep sinks are draining at the same time, floor sink is over flowing. Manager states they are making sure that they drain only 1 prep sink at a time. [COS] Ensure ALL employees know this and do not let floor sink over flow.

Site Observation:
> LOTS of small drain flies at dishwasher area and some are at buffet hot holding line. [CA] Eliminate drain flies from food facility.
> Found one dead roach inside cabinet under soda dispenser. [CA] Clean up dead roaches.
++ Note: Last pest control service was on 5/16/16.

High temp dishwasher is not working. Per manager, booster heater is in disrepair. During time of inspection, observed an employee run dishes and submerges all utensils in 200ppm QUAT sanitizer. Per manager, part for booster heater has been ordered. [CA] Maintain equipment in good repair.

> Several broken/missing tiles and worn out grouts on floor, thus water and food debris are stuck, making it unsanitary condition.
> Ceiling above high temp dishwasher, especially where built in hood is penetrating ceiling panel is CRACKED, holes are noted. Ceiling condition at this location does not look safe/stable.
